Disability Support Worker

Southern Cross is seeking motivated, caring and committed individuals as Disability Support Workers to deliver care and support to a 41-year-old man who is ventilator dependent and living in his own home.
This support will enable the person with a disability to remain living in their home whilst receiving assistance for all of their everyday living tasks and achieving their person centred and therapy goals.
Support Workers will receive support to achieve the skills required to deliver individualised support to people with disabilities living in the community.
Duties may include:
Ventilation and Tracheostomy Management
Assistance with Personal care tasks
Medication administration
Assistance with Therapy tasks
Use of Mobility equipment – e.g. hoist, wheelchair, shower chair
Light Domestic assistance
Meal preparation
Community participation – e.g. medical appointments, Shopping and/or social outings
Service delivery times
This Program will require services to be provided at specific time
7am to 3pm Tuesday fortnightly
Active overnight 11pm to 7am Friday weekly and Sunday fortnightly
Requirements
6 months experience in disability care preferred
current references
mobile phone
current drivers licence and reliable vehicle
As part of the engagement process, all successful applicants must receive a National Criminal History Clearance and need to obtain a Working with Children Clearance to commence working as a Support Worker.
